Notes

• The form MC-7-1 with your supervisor's seal can be uploaded only to that file.
• The form MC-7-1 without your supervisor's seal must be uploaded with a file of the copy of an approval email from your supervisor.
• The file name should be "Student ID number_MC-7-1_name.pdf".
• The form MC-7-1 can also be downloaded from WebClass or the "News & Events" page of the Graduate School of Science and Engineering website.
